Adding CVD requirement to online payment form

Greetings. I have the CVD requirement enabled on my account, but I don't know how to add the CVD requirement to the form that a customer needs to complete when they make a payment online. I urgently need to add this. Can anyone please provide assistance on this? Many thanks, in advance!

    I have done as you indicated. I had already emailed eProducts@moneris.com yesterday, but I'm not sure whether the message made it through, as I didn't receive any confirmation auto-response.
